Public Holiday Closure

The health centre will be closed on Tuesday 5th June 2012 for the Diamond Jubilee Bank Holiday. 

We will be open as normal on Monday 4th June and reopen on Wednesday 6th June at 8am. 

During the closure, should you require urgent medical advice, please contact NHS 24 on 08454 24 24 24

If you require a routine appointment, repeat prescription, or your call does not require immediate attention, please contact the surgery when we reopen. 

Information for new patients considering registration with our practice

The practice is currently accepting new patients. That means that our list is open and we will be happy to register any patients who live within our catchment area and provide them with General Medical Services.

Rumbling Bridge

If the area you stay in is not listed above, please contact the practice and we will advise whether your area is covered or not. We can be contacted on 01259 742120.

Registration process

Registration with the practice is easy. All you need to do is pop in and collect the necessary forms, complete them and return them along with an appropriate form of identification and we'll do the rest. Alternatively, you can download the forms from the links at the bottom of the page. As soon as you have completed the forms and returned them to us, you can then make appointments as required. A new patient medical is not compulsory and is not required unless you specifically request one. 

Transferring your medical records

Your medical records will be transferred from your previous practice and that usually takes around about 2 to 3 weeks if you're transferring from another Scottish practice. If you're transferring from England or Northern Ireland, it can take a bit longer. If you have come to this country from out-with the United Kingdom, we won't get any records for you. You don't need to do anything to transfer your records. We'll do it all for you.


Acceptable forms of identification are passports, driving licences or birth certificates. If you haven't got access to appropriate identification, please inform reception at the time of registration who will advise on what you should do. We will be able to register patients without identification in special circumstances.

Finalising the process

That's it. Once we have your records the transfer process is complete. In the main the transfer process will remain seamless and you should experience no hiccups when transferring surgery. The most you'll need to do is complete a couple of forms and provide us with identification. Once we have your records and we've summarised them we invite you to inspect the summary we have done and advise us of any inaccuracies or omissions. No one knows you better than you do and if there's something missing from your medical records or your summary we'd like to know about it! This process is not compulsory but we invite everyone to take up the offer. You can indicate your desire to do so if you wish when completing the registration pack.

Once you're ready to register, you can download a registration pack here by clicking on the links below. There are 3 packs - One pack for children up to their 5th birthday, one for children aged 5 to 14 years old, and one for all persons over the age of 14. Please make sure you pick the right one!

Once you've downloaded, printed and completed the registration forms, please return them to the health centre with your chosen form of identification, photographic being the preferred option.
