Chronic Disease Management

People with chronic diseases, or long term medical conditions often need regular monitoring and review.   This is generally organised and led by the practice nurses.   Most people will be invited for a review on an annual basis around the time of their birthday.

Where possible you will be invited for an appointment for blood tests and appropriate measurements (e.g. weight or blood pressure) a week or two ahead of your annual review.  This way it is possible to consider and discuss the latest information at the clinic appointment.

Appointment Type

As you are aware there is a national emergency in the health service due to Coronovirus (COVID-19) and as a results we cannot currently provide a "business as usual" service. In line with the national NHS guidelines, may of our face-to-face appointments have been moved to telephone appointments and near me video consultations.

Your Chronic Disease Review will now be dealt with over the phone with one of our Practice Nurses. If you require bloods then an appointment will be made with one of our Healthcare Assistants. If you require blood pressure, height and weight readings, health status information please take these at home and bring the information to your appointment with the Healthcare Assistant or email it to FV.DollarHC@nhs.scot. The letters below will guide you to what is required for your review.
